Couldn't be a jumped tooth because starting would be hard cold or warm.

It is absolutely essential for the exhaust system to be leak free at all points between the exhaust manifold gasket and the catalytic converter. Get new gaskets and weld anything that's cracked.

Also essential for the ignition timing not to be off by more than 1-2 degrees.

Cat might be clogged from years of being overloaded with heavy emmissions caused by the exhaust leaks and screwed up timing although your fuel economy would be poor too so maybe your cat is still OK
